The County Health Rankings and Roadmaps program is a collaboration between the Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute. The County Health Rankings provide the rank of health of nearly every county in the nation and help counties to understand what influences how healthy residents are. Measures available include high school graduation rates, teen births, income, unemployment, and obesity. The County Health Roadmaps are assisting communities bring people together to assess the multiple factors that influence health, collaborate with other communities, and implement lasting changes in order to improve health.
